HPS collects personal information about you and your family in various ways such as through the use of our websites, through benefit enrollment applications and through interaction with our partners who facilitate the delivery of your healthcare. We may collect the following types of personal information (as permitted by applicable law):

Personal information (name, address, telephone number, email address, Social Security number, date of birth and your response to similar background information)
User name and password when you register on our site
Payment information (such as credit card number, expiration date, authorization number or security code)
Protected Health Information (procedure or services performed, name of care provider, name of facility, date of visit, etc.)
HPS is committed to protecting your personal information and will request to verify your identity when you reach out to HPS to reduce the risk of unauthorized disclosure.

Minors

We do not knowingly collect Personal Information (as defined above) from persons under the age of 18. If you become aware that a minor has provided us with this type of information, please contact us to have the account deactivated by sending us an email at legal@hps.md or by calling at 888.477.7968.

Please note that we collect and store certain limited information related to your visits and interactions with our Site. What we collect and how we use it is set forth below.

What we collect if you only browse our Site: If you do nothing during your visit but browse, read pages or download information, we will gather and store certain limited information about your visit. We use Hotjar in order to better understand our users’ needs and to optimize this service and experience. Hotjar is a technology service that helps us better understand our users experience (e.g. how much time they spend on which pages, which links they choose to click, what users do and don’t like, etc.) and this enables us to build and maintain our service with user feedback. Hotjar uses cookies and other technologies to collect data on our users’ behavior and their devices (in particular device’s IP address (captured and stored only in anonymized form), device screen size, device type (unique device identifiers), browser information, geographic location (country only), preferred language used to display our website). Hotjar stores this information in a pseudonymized user profile. Information We Share

Healthcare technology companies, including HPS, need to share Personal Information in order to run their everyday business. We do not authorize these vendors to use or disclose your personal information except as necessary to perform services on our behalf or comply with legal requirements. Keep in mind that Health Payment Systems may still continue to send email communications or call you in order to provide a product or service that you request.

Notice to California Residents

Subject to certain limitations, California residents may request to be provided with a list of certain categories of personal information that we have disclosed to third parties for their direct marketing purposes during the immediately preceding calendar year, and the identity of those third parties. How We Protect Your Personal Information

We closely safeguard the integrity of our Site with the intent to prevent unauthorized access to information maintained by us.
